Job Description:
A company is looking for a Care Navigator (Temporary, with
Potential Conversion to Long-Term Role).
Key Responsibilities:
Provide high-quality support to members for their gender journey
and health outcomes
Manage resource navigation and care coordination for members with
complex needs
Respond to member inquiries and mental health crises, ensuring
timely and empathetic support
Required Qualifications:
1+ years of experience working with the trans community in a
healthcare or community setting
2+ years of experience in patient navigation, social work, or care
coordination
Experience with electronic health record systems and scheduling
platforms
Understanding of trauma-informed care principles and motivational
interviewing
Basic knowledge of computer applications such as Microsoft Office
and video conferencing tools
